Insurance and Patient Information
What information do I need to be seen by a practitioner? You'll need a valid photo ID and insurance card (if you're paying with insurance). At the time of your visit you will complete an easy registration form on a tablet and then be seen by the practitioner. We don’t take appointments; just walk in when you need us.
