What to Expect During Your Visit
If you are new to Cornerstone, you can feel confident in our care from the time you enter our front doors until you check out. Please remember to bring a copy of your insurance card and ID to your appointment. You will be sent several reminders about your upcoming appointment, including an email three days prior to your appointment which will contain a link to fill out your new-patient paperwork. It is very important that you complete this paperwork in advance. Doing so will enable us to provide you with the best, most comprehensive care. If you are unable to fill out this paperwork at home, please arrive to your appointment 45 minutes early to complete it at the clinic prior to seeing your provider.
After parking, please walk through the front doors on the main level of the Cornerstone building. There will be someone at the front doors to greet you, or there will be signs posted to direct you to check in with your particular provider. Our new state-of-the-art waiting room is equipped with USB ports and outlets for your convenience. We hope you find this atmosphere relaxing and friendly while you finish the check-in process.
