Schertz Medical Home to accommodate influx of active duty service members, families
The Schertz Medical Home was designed to provide the necessary health care for service members and their families residing in the Schertz community. Because of the increasing numbers of people relocating as a part of 2005 Base Realignment and Closure law, the Army has implemented 17 off-post primary care clinics in 11 different communities across the nation to provide better service and access to health care for active duty service members and their families.
Facility Expansion and New Location
The Schertz Medical Home is moving into a larger facility to meet the growing needs of its patient population. A ribbon cutting ceremony will be held at the new location, 17115 Interstate 35 North, Suite 123. The goal of moving to the new location is to grow, expand and be able to see a larger population of TRICARE Plus patients.
At this time we currently have just over 10,000 patients enrolled. Moving to the new location, we will have the capacity to serve more than 15,000 patients with a new third team which includes a Pediatrician. In addition, the new physical therapy space will offer a large open gym with state-of-the-art equipment.
Comprehensive Health Care Services
The patients of Schertz Medical Home will receive comprehensive health care by a team of doctors and nurses to ensure their health and wellness by delivering preventive screenings and services, taking care of new health concerns as they arise and managing chronic health problems. The new location will provide the same services to include an expanded physical therapy clinic and a drive thru pharmacy option for phoned-in non-controlled substance refills.

- Chronic disease management
- Preventive health care services
- Urgent care
- Laboratory and pharmacy
- Licensed behavioral health professional to provide marriage, family and individual counseling
Each family practice team assigned to the Schertz Medical Home will have a nurse case manager to follow patients with chronic illnesses and a triage nurse who will facilitate and enhance communication with patients through e-mails and messages from appointing agents. This team will also have a care coordinator, a licensed practical nurse who will track referral and patient care received outside of the Schertz Medical Home.
Enrollment and Clinic Operations
TRICARE Plus (over age 65) and TRICARE Prime beneficiaries are eligible for enrollment at the Schertz Medical Home. Currently, the Schertz Medical Home Family Medicine Clinic is accepting active duty family members, TRICARE Prime retirees and retiree family members ages 0-64 years old. Another benefit is a much larger waiting room with specific waiting areas for the PCMH clinic as well as pharmacy and lab. Patients will no longer have to share a room when waiting for their medications or appointments.
