Neighborhood Health Services Corporation: COVID-19 Vaccination and Testing Services
The Mission of the Neighborhood Health Services Corporation is to be a leader of ambulatory primary health care services within the medically under-served and uninsured populations in the communities we serve. We offer quality services in a cost effective, safe environment that is culturally diverse and responsive to the changing needs of the patient population.
Vaccination and Testing Schedules
Neighborhood Health (NH) provides multiple locations and times for patients to receive services and Register for COVID-19 Vaccine:
- Annex Building: NH will be offering COVID vaccines at our annex building, Mondays and Wednesdays from 9:00 AM to 01:30 PM / 02:30 PM to 04:00 PM.
- E-port: Covid-19 vaccines at E-port on Friday 03:00 PM to 04:30 PM.
- Testing in Plainfield: NH will offer COVID testing (Plainfield) across the street on Rock Ave, Mondays and Wednesdays from 2:30 pm- 4:00 pm.
V-Safe Program and Registration
V-safe Registration is a vital step for those who have been vaccinated. V-safe is a smartphone-based tool that uses text messaging and web surveys to provide personalized health check-ins after you receive a COVID-19 vaccination. Once you have received the vaccine you need to use the link below to register for V-safe.
Through the V-Safe Program, you can access the following benefits:
- Through v-safe, you can quickly tell CDC if you have any side effects after getting the COVID-19 vaccine.
- Depending on your answers, someone from CDC may call to check on you and get more information.
- And v-safe will remind you to get your second COVID-19 vaccine dose if you need one.
