Directions: We are located at 200 Theda Clark Medical Plaza located on the Theda Clark Medical Center Campus. Free valet parking is available at the main entrance. Enter at the main entrance of the hospital and turn right into the Physician Office Building. Take the elevator to the second floor. O ur office is on the right when getting off the elevators, Suite 270.

Directions: We are located at 2500 E. Capitol Drive in Appleton. Exit Hwy 41 at Ballard Road and turn left on Capitol Drive. The Encircle Health building is on your left. There is free valet parking. Enter the building through the rotating doors and skip the main registration. Take a left going down the West Gallery hallway. We are the second office on the right connected to Fox Valley Surgical. Please enter the doors and check in at our appointment desk.

Directions: We are located on the Appleton Medical Center Campus at 1818 N. Meade Street, Suite 330. There are a couple areas where you may park and enter the building to get to our office suite. Recommended: Enter the parking lot off Glendale Avenue and park in the lot near the Appleton Heart Institute. Enter the Medical Office Building West, at door #38. At the end of the hallway on the right, take the elevator up to third floor. Alternate: Enter the parking lot off Grant Street and park in the hospital parking lot. Free valet parking is available at the West Entrance. Enter the West Entrance at door #38, go past the information desk and turn left to the Medical Office Building West. Continue to the elevators. We are on the third floor.

Directions: Please enter through the Main Entrance (across from the Parking Ramp) of St. Elizabeth Hospital. After entering the building, stay on the right side and go past Starbucks and the Gift Shop until you reach a hallway that is on your left (just beyond the fireplace and restrooms). Go straight and take a quick right to the Madison Elevators. Take the Elevator to the 3rd Floor OB/GYN Department. Recommended: Park in the parking ramp across from the main entrance of St. Elizabeth Hospital. This is located on Oneida Street. Enter the main entrance. There are signs for the Madison St. clinic on the ceiling. Follow the M (Madison Street) signs to the Madison Street Elevator.
